How to Get a Connecticut Medical Marijuana Card

The state of Connecticut offers Medical Marijuana cards through the Department of Consumer Affairs. There is an online system where applications must be submitted.

Who can qualify?

To be eligible for a Medical Marijuana Certificate you must be a resident of Connecticut and 18 years of age or older, or have a parent or guardian who can register as a caregiver. You cannot be an inmate in a correctional institution or a facility supervised by the Connecticut Department of Corrections. And you must receive one of these diagnoses from an approved physician:

  • Cancer
  • Glaucoma
  • HIV or AIDS
  • Parkinson’s disease
  • Multiple Sclerosis
  • Spinal cord damage with with Objective Neurological Indication of Intractable Spasticity
  • Epilepsy
  • Cachexia
  • Wasting Syndrome
  • Crohn’s Disease
  • PTSD
  • Sickle Cell Disease
  • Post Laminectomy Syndrome with Chronic Radiculopathy
  • Severe Psoriasis and Psoriatic Arthritis
  • Amyotrophic Lateral Sclerosis
  • Ulcerative Colitis
  • Complex Regional Pain Syndrome
  • Cerebral Palsy
  • Cystic Fibrosis
  • Irreversible Spinal Cord Injury with Objective Neurological Indication of Intractable Spasticity
  • Terminal Illness
  • Uncontrolled Intractable Seizure Disorder

If the patient is a minor they must receive one of these diagnoses:

  • Cerebral palsy
  • Cystic fibrosis
  • Severe epilepsy
  • Terminal illness
  • Uncontrolled Intractable Seizure Disorder
  • Irreversible Spinal Cord Injury with Objective Neurological Indication of Intractable Spasticity

How much does it cost?

There is an application fee of $100.00 for patients and $25.00 for caregivers

How do I apply?

To apply for a Medical Marijuana card do the following:

  1. Visit with your physician and have them file an online certification
  2. Create an account with the DAS Business Network to access the online certification system
  3. Log in to your account and follow the registration process, providing your identification information
  4. Mail or upload one of the following: proof of identity, proof of residency, or a current passport sized photo
  5.  Pay the fees online  or mail a check or money order made payable to “Treasurer, State of CT”
